Learner Outcomes: Students will realize the implications of smoking on their health by informing peers, parents and coaches of the dangers of teen smoking.
Duration of Lesson: One class period (45 minutes) to disperse, read and discuss handouts.
One day outside of class to collect signatures.
Materials: You(th) and Tobacco Handout--one per student
Procedure and Signature Collection Sheet--one per student
Technology Tools/
Courseware:
Computer with Internet access.
Teacher Notes: Secure computer laboratory access. 
Make sure that all students have signed county/school Acceptable Use Policy. 
Print out the handouts listed below in Adobe Acrobat Format - ( PDF-629K)--be patient--the Adobe version takes a few minutes to download from the CDC  site but is easy to use and print.  You may also wish to stress to students that papers handed in late will have a deduction applied. 
Procedures: 1.  Access the Center for Disease Control web site, You(th) and Tobacco.  Use the Adobe Acrobat Format link to get a printable copy of: "What You Should Know   About Tobacco", "Parents-Help Keep Your Kids Tobacco-Free", and "Coaches — You Can Influence Youth".  Print one copy of each per student. 
2.  Give each student a Procedure and Signature Collection Sheet to provide instruction and also collect the required signatures for this activity.
. Modifications: All IEP modifications should be followed. 
Enrichment Activities: Students can take the Tobacco Quiz  provided by the Center for Disease Control and self check their answers.
Evaluation/
Assessment: 
Students who collect all 6 signatures should receive 100% of the points possible.
Students who collect 5 signatures should receive 83% of the points possible.
Students who collect 4 signatures should receive 67% of the points possible.
Students who collect 3 signatures should receive 50% of the points possible.
Students who collect 2 signatures should receive 33% of the points possible.
Students who collect 1 signature should receive 17% of the points possible.
