"The Survey Says...."

Grade Levels:        9-12

Subjects:               Health, Physical Education, Business Computer Literacy

Learner Outcomes:

1)    Upon completion of the lesson, the learner will gain an understanding of
the level of tobacco usage (cigarette smoking) in our school.
2)    Students will create and format an Excel chart based upon survey data.

Duration of Lesson:  90 minutes

Materials: 

Chalkboard or dry erase board with writing utensil

Grid for recording survey results
Technology Tools/Courseware:
Computer for each student
Internet Access
Microsoft Office Excel 97 version or above
Printer
Projection Device
Teacher Notes:

Secure computer laboratory access.
Make sure that all students have signed county/school Acceptable Use Policy.
Students should have completed surveys prior to coming to class.

Procedures:

1)    Have students to take out their five completed school tobacco surveys.

2)    Give each student a Grid for recording survey results.
3)    Lead a group discussion on the results of the survey.
4)    List the results of the survey on the board from each question.  Be sure that all areas on the grid are completed.
5)    Give students a review in creating charts in Excel.  (The Microsoft website offers a Microsoft Excel Tour.  If necessary, create the first chart together as a class.) **If creating the first chart together, be sure to have projection device in lab.
6)    From the results of the survey, students will create five Excel charts.  One chart will be based upon the ratio of female to male smokers:  The other charts will be based upon the number of smokers in each grade level; one chart for each grade level based upon the ratio of the number of students surveyed to the number of student smokers.

Modifications:

Possible special education modifications include the following:

For physically impaired students:

If available, use a touch screen.
Use a trackball or IntelliMouse.
Have templates available with the basic structure of the spreadsheet already keyed.
For mentally impaired students:
Reduce the number of surveys they must complete.
Limit the number of Excel charts to complete.
Allow for additional time when necessary.
When necessary, place students in cooperative learning groups.
For visually impaired students: 
Provide large print worksheets.
Increase the zoom in Excel.
*Consult with Special Education Department regarding specific IEP requirement.
